Quiz Creator

Hi,

I am devising a safety quiz for my Dept and was wondering which software would you recommeded for quiz creator.

I would like the user to finish the quiz and the results for the quiz be send to me....

It is a light event, so that i can give some small prize to the top scorers...

I have tried the html ones on the web, but they seem to lack the send results to me function...Thanks.

I'd like to here more suggestions for this too.
For surveys, one of the best known tools is: http://www.phpsurveyor.org/ but that's not really well suited to quizes.
