tiny standalone relational database

Is there a standalone relational database out there?  Just a self-contained, portable db.  Maybe it could rely on json or csv and text files?  I'm looking for one with a simple form-builder included.  I don't want to go so far as Access or the Office-suite dbs.

I'm thinking something more like TablePro or Skwire's SnapDB except those are only spreadsheets, not dbs.

Free to use, small, portable (if you want it to be) and available for all the main operating systems.

Shades - SQLite is good enough, and I've used before as a datbase, but I want to find a form-builder to go with it.
