I'm not familiar with many PIMs but surely there's a bunch out there that you can link files to - or simply drag-drop them in there.
I'm also not sure what other capabilities you're looking for (?)

Rereading, it sounds like you want a hierarchical template of sorts within the PIM - or would you prefer to keep the folder/file structure and link them to the PIM file?

What have you tried & why didnt you like them?

I believe InfoQube (sqlnotes) will do what you want and is very 'extensible' in terms of using that info and relating it to other info etc., it's portable too (but is still in beta). I use it myself mainly to keep track of files I'm working on. I'm sure there's others, but I think you'll need to give some more details.

(Isn't there a simple notetaker that saves notes as text files - locate those text files you could possibly just duplicate them as a template for the next client - would something that simple do? If so, I'm sure we could track it down)

You might look into Wikidpad. It is a text based wiki. You can create a new wiki project per customer, thus you might be able to produce what you are looking for. Every wiki entry will be saved to your hd as a human readable text(wiki) files.
As far as organizing files with explorer goes, instead of using multiple explorers you can try this one http://www.softwareok.com/?seite=Freeware/Q-Dir. It is a quad-pane explorer

I have tried numerous ways methods\programs, I have looked at sqlnotes\tiddlywiki\treesheets\infocube\mempad\neopad\yourtree\infowonder
I have basically tried a lot which have been in the PIM\info managers\note taking threads

I found most of them don't suit what I need or a Little to bloated, I found sqlnotes I little over whelming when I looked at it a while ago, thats not to say they aren't good, just i haven't found one which works or fits\suits in with my work-flow

Q-Dir - is great been using it for the Past year, I think it really helps with productivity, I will have another look at wikipad, I sometimes miss using q-dir when working on a customer site

@ tomos - something as simple as that would do, but I think im basically what im doing now, what im doing now just trying to manage it a bit better
I have a customer area and template area, I copy the template folder and then rename the folder to the customer name
