I thought it would be a good idea at the end of fundraiser month to have a little survey we ask people to fill out, to solicit opinions about what the site should focus on, etc.

I'd like to ask for suggestions about the kinds of questions it would be useful to ask.

Please suggest some questions!

I like having the "any questions we didn't ask but you'd like to answer anyway" question - or response option at the end of many smiley

I am not claiming these questions are very insightful or original but still

"Does donating affect the number of visits you make to the site, now and later and by how much?"
"Does it inspire you to learn programming and help others?"

And the answer to this may be obvious or is it?

"Did one particular DC program attract you to donate?"

Keep 'em coming
(note that this survery won't be just for donators -- i suppose a natural question to ask might be, if you have chosen not to donate -- why not?)

And if not, what aspect of the site DID you like?

I think that's a good question to ask everyone, not just those who didn't donate.

Good point! (If nothing else that'll get me banned.  smiley)

It's important to kick around ideas for questions and also to think about what actions would be feasible based on the answers. Then the questions can be defined with as much precision as possible.
A bunch of loose, open-end responses will be difficult to categorize (though they may contain a one-off comment that turns out to be the best idea ever proposed).

"Now that you have donated - what do you expect to change/enhance/improve/happen?"
"Would you like to be able to donate more than you have?"
"Do you know your karma increases by donating but not by the amount donated?"
"Do you see the DonationCoder community as a role-model for other activities (kindergarten care, car repair, shelter for homeless...)"
"Have you smiled today?"
