One of the extensions in Firefox I can't live without is "Back to top":

One rightclick, and one click to go to top of page. Done!

This is done without almost no mouse movement. Of course you can also use it for going to the end of the document.

There are some options if you want to adjust its behavior.

Tiddlywiki is an interest concept you should checkout, even if it doesn't comply to all your demands. There a long list of fantastic Tiddlywiki plugins at the Tiddlytools website. There seems to be a tree list view plugin called RelatedTiddlersPlugin.

Another good source is Tiddlyvault.

If you want to check out the many differents adaptions - have a look at the list at wikimatrix.org.

Edit:
I did a quick googling for "tiddlywiki tree list plugin or macro" and found a plugin called "TagsTreePlugin"

I downloaded and started it. No problem.

Are you using the latest version (ver 2.28.3)?

Under the top menu, you should have three tabs: Search | Directory | Catalog.
